Beyond factor H: The impact of genetic-risk variants for age-related macular degeneration on circulating factor-H-like 1 and factor-H-related protein concentrations.

AbstractAge-related macular degeneration (AMD) is a leading cause of vision loss; there is strong genetic susceptibility at the complement factor H (CFH) locus. This locus encodes a series of complement regulators: factor H (FH), a splice variant factor-H-like 1 (FHL-1), and five factor-H-related proteins (FHR-1 to FHR-5), all involved in the regulation of complement factor C3b turnover. Little is known about how AMD-associated variants at this locus might influence FHL-1 and FHR protein concentrations. We have used a bespoke targeted mass-spectrometry assay to measure the circulating concentrations of all seven complement regulators and demonstrated elevated concentrations in 352 advanced AMD-affected individuals for all FHR proteins (FHR-1, p = 2.4 × 10 ; FHR-2, p = 6.0 × 10 ; FHR-3, p = 1.5 × 10 ; FHR-4, p = 1.3 × 10 ; FHR-5, p = 1.9 × 10 ) and FHL-1 (p = 4.9 × 10 ) when these individuals were compared to 252 controls, whereas no difference was seen for FH (p = 0.94). Genome-wide association analyses in controls revealed genome-wide-significant signals at the CFH locus for all five FHR proteins, and univariate Mendelian-randomization analyses strongly supported the association of FHR-1, FHR-2, FHR-4, and FHR-5 with AMD susceptibility. These findings provide a strong biochemical explanation for how genetically driven alterations in circulating FHR proteins could be major drivers of AMD and highlight the need for research into FHR protein modulation as a viable therapeutic avenue for AMD. Factors affecting mental health support to the British Armed Forces: Part OneFinnegan, Alan; Finnegan, Sara; Thomas, Mike; University of Chester (Ptm Publishers Limited, 2014-11)To help the British armed forces minimise mental health problems while undertaking military duties, operational psychological support is provided by military mental health nurses. This series of two articles is part of the first qualitative research completed in Afghanistan by British armed forces into the effectiveness of the military mental health nursing role. The authors aim to increase understanding of the factors that affect the delivery of nursing care during an operational deployment, including educational and clinical competency, multiprofessional and multinational boundaries, and the challenges of providing nursing care for both military personnel and local nationals. Assessing Risk Factors for Reproductive Failure and Associated Welfare Impacts in Elephants in European ZoosHartley, Matt; University of Chester (European Association of Zoos and Aquaria, 2016-08-02)Reproductive failure in elephants is thought to be caused or influenced by a range of factors such as obesity, infectious disease, husbandry, facilities, stress, behaviour, maternal experience, herd size and social grouping. Due to the low reproductive activity of the small zoo elephant population, scientific study into the relative importance of these factors is limited. This study takes an epidemiological approach using risk analysis methodologies to collate information from expert opinion, data set analysis and a targeted questionnaire to identify and assess a range of physical, behavioural and husbandry based risk factors, which may affect reproductive success in elephants housed in European Zoos. Much of our knowledge on reproduction in zoo elephant populations originates from North America where there are significant differences in herd structure, management practices, climate and mean age. By combining multiple sources of evidence including a large survey of reproduction in the European elephant population and eliciting expert opinion from scientists, zoo managers, veterinarians and keepers working with European zoo elephants in a structured, transparent and scientifically recognised process it has been possible to identify the most important causes of reproductive failure and assess the influence of a range of potential confounding factors. Important causes of reproductive failure included lack of access to a compatible bull, herd stability and compatibility, allomothering or maternal experience, management practices at parturition and the impact of Elephant Endotheliotropic Herpes Virus. This work is to be used in the development of evidence-based elephant management and welfare recommendations and highlights priority areas for further research.
Factors affecting mental health support to the British armed forces: part twoFinnegan, Alan; Finnegan, Sara; Thomas, Mike; University of Chester (Ptm Publishers Limited, 2015-01)The first part of this series (JCN, 28(5): 30–32) provided the background to the study, which used semi-structured interviews with 18 nurses based in Afghanistan during 2013 to focus on factors affecting the delivery of mental health care in the field. This, the second part of the series, details the results of the study in the form of analysis of the interviewees’ verbatim transcripts. The study offers an insight into the role of deployed mental health nurses and examines some of the challenges they face. The findings demonstrate that managing the mental health of armed forces personnel on an operational deployment requires the ability to develop trusting relationships, identify factors leading to stress, and help staff to feel supported.
